Liability Waiver & Emergency Medical Consent

 I acknowledge that I am enrolling my child in a program offered by Prolific Creation Studios LLC. I understand that while all reasonable safety precautions will be taken by staff and instructors, participation in any enrichment program involves inherent risks, including but not limited to physical activity, outdoor exploration, art or craft supplies, and interactions with peers.

I, the undersigned parent or legal guardian, hereby release and agree to hold harmless Prolific Creation Studios LLC, its owners, directors, employees, instructors, volunteers, and any partnering locations from any and all liability, claims, demands, losses, or causes of action that may arise from my child’s participation in the program. This includes any accident, injury, or illness that may occur before, during, or after program hours, whether on-site or during off-campus activities.

In the event that I cannot be reached in an emergency, I authorize the staff of Prolific Creation Studios to secure appropriate medical care for my child, including transportation to and treatment at a hospital or emergency care facility. I understand that all efforts will be made to contact me before any emergency treatment is provided.

At Prolific Creation Studios LLC, we strive to cultivate a respectful, inclusive, and safe learning environment for all students, families, and staff. We believe that strong character, kindness, and cooperation are essential to a successful educational experience.

By enrolling in this program, students and families agree to the following expectations:

• Treat peers, instructors, and staff with respect, kindness, and courtesy.

• Participate cooperatively and attentively in all activities and class discussions.

• Use appropriate language and behavior at all times.

• Show respect for the learning environment, materials, and shared spaces.

• Refrain from any form of bullying, intimidation, or discrimination.

• Follow safety rules, remain within designated areas, and behave responsibly.

• Work through challenges or disagreements in a peaceful and respectful manner, seeking adult support when needed.

We are committed to supporting students in learning and growing from mistakes. However, repeated disruptive, disrespectful, or unsafe behavior that interferes with the learning or safety of others may result in a meeting with parents, temporary suspension, or dismissal from the program at the discretion of the directors.

Attendance Expectations

• Consistent attendance is strongly encouraged for continuity of learning, group cohesion, and full participation in projects and discussions.

• If your child will be absent, please notify us in advance by emailing info@prolificcreations.org.

• Missed classes are not refundable or credited, as staffing and resources are planned in advance.

• Illness and emergencies are understandable, but we ask families to make every effort to attend scheduled classes.

Withdrawal Policy

• A 30-day written notice (sent to info@prolificcreations.org) is required to withdraw.

• Tuition is billed quarterly (every 3 months). Early withdrawal will be reviewed on a case-by-case basis.

• Tuition already paid for the current quarter is non-refundable unless otherwise approved by Prolific Creation Studios LLC.

Billing & Payments

• Families are invoiced quarterly unless alternate arrangements are made.

• Invoices are sent via email; payments are due by the date listed.

• Late payments may result in late fees or suspension of participation.

• Monthly payment plans are available upon request.

Cancellations

• Our studio has limited seating, and many lessons sell out.

• Please cancel at least 24 hours before your scheduled lesson by calling or texting 520-901-0302 (10 a.m.–3 p.m.).

• With proper notice, payment will be credited toward another lesson.

• Lessons or classes canceled without 24 hours’ notice are non-refundable and not credited.

Payment Policy

• Lessons must be purchased before a time slot is reserved.

• Reserved dates without payment may be released to another student.

• If you are approved to pay after a lesson, payment is due within 3 business days.

  • A $25 late fee applies to overdue payments.

  • Unpaid balances may result in canceled future lessons.

Behavior Policy

• Instructors may cancel lessons or enrollment without refund if inappropriate behavior is exhibited before, during, or after lessons.
